What state form requires a child's SS# for a day care provider to do their taxes? None that I know of. The only persons SS# or bussiness number needed should be the daycares. for the parent to claim it on there taxes as a deduction......BEWARE.. Nothing requires a child's SS number. Don't give it out - almost sounds like a scam or they want to use it for some illegal to do their taxes with. Too many kids are getting their first job and find out that they have been working for years. Every state is different. Search for forms on your state dept of revenue homepage. The federal form is a IRS Form 2441 if you want to claim the child care benefit. The state form depends on the state you live in. Unless your state has some unusual requirement I don't know why the day care provider would need the SS# of your child unless it is to provide you a receipt for the amount that you paid for that child. Your question either is or is not. Does that make sense? Your question does not either. If you pay for daycare then who ever does your taxes should know. Don't do the taxes yourself. If it cheap to hire a good tax person. If you run a daycare center then again do not do the taxws yourself. Hire a good accountant to watch over your business. Well, if you are receiving assistance from the state to pay for daycare, they may need it for that. I've never given my daughter's out though, and I did get child care assistance while I was in school. Otherwise, you could tell them that you need to see the form they are completing and tell them you will have your "accountant" complete it.
