There are a great many options for the employer to choose from. Some employers even choose to offer multiple plans to cater to their employees needs/preferences, for example an employer could contract with a health insurance company to offer health insurance with several different deductible levels (in general the higher the deductible, etc the lower the premium to the employer/employee).
