They will have to become permanent residents but do not need to become citizens.

The waiting period to get on Medicare is 5 years. They will have to pay both the Part A and Part B premium (currently over $500 per month but sure to go up in 5 years) plus any premium for a supplement or Medicare Advantage plan.

If they are over age 60 there are some plans available to them and a fairly reasonable cost. A 60 year old will pay $218 and up per month. Contact an independent agent that works in the senior market for more information on this type of plan.

Otherwise they'll need to get on an individual policy and can expect to pay upward of $1000 per month for both, more if they've got health issues. Again, an independent agent will be able to help them find the right policy and can also work for them if there's any problems. Source(s): Independent Agent
If they want to retire to the USA they will, by definition have to become legal permanent residents, so they will be able to purchase health insurance. It won't be cheap though and they will not qualify for medicare straight away. I believe the waiting period is five years.
I'm not quite sure if they will be able to get insurance. But just make sure they come here illegally and they won't need insurance. The medical care is free then.
They will have to become permanent residents as far as I know. You may want to check online. You may want to try a website that compares multiple companies at once to get you the best price. I am paying less than 陆 after I did.
