Is there a law about health insurance stating that there HAS to be an in-network?

provider within 20 miles of where you live?

Nope, but your company will probably not choose a plan that puts very many of their employees in that situation, so I am going to assume that you are a rare case in your company. If not, talk to your HR department. Otherwise, call your insurance company and ask them to transfer you to a care co-ordinator. Explain the situation, and ask them to make a one person exception for your provider of choice. If they won't, speak to your HR department, and get them to call their account person at the insurance company. The insurance company does not have to do this, and might not, especially if there is an in-network provider 21 miles from you or something. If it's really a hardship, though, they probably will. When you signed up for the insurance, you should have read the fine print. The insurance company has every right to limit payment to in network, but it has to be in the contract before you sign up.
